2025 Wellness Program

Employees enrolled in the Medical Plan can earn a medical premium discount or Increased HSA Contribution in 2026.  Please read below to learn more about your 2025 Wellness Program!

Please complete Step 1 and 2 listed below by SEPTEMBER 30, 2025 to earn the incentive

Access all of your 2025 program details, including the 2025 Program Guide, via the Wellness Locker on the Wellness Portal and mobile app.

Step 1: Complete the Know Your Number Assessment & Self-Report Metrics

Step 2: Complete One (1) Wellness Activity

Log Into Your Wellness Portal/Mobile App

You will have access to your very own personal Wellness Portal. Credentials have already been generated for new account users – otherwise, existing participants will continue to use their current credentials. Please follow the steps below to log into your Wellworks For You account. In accordance with confidentiality laws, your individual data is accessible only to you and the third-party vendor, Wellworks For You. 

1. 1. Go to www.wellworksforyoulogin.com or download the mobile app for access to the Portal
   2. Your account credentials are listed below:

**PLEASE NOTE: The temporary password is only for the first time you access the Wellness Portal, and you will be prompted to change it upon entry. If you have accessed the Wellness Portal in the past, you should continue to use your existing password.

Forgot Your Username and/or Password?

1. Select the Forgot Username or Forgot Password links on the homepage.
2. Enter your email address to initiate the password reset or username retrieval process
